That’s why we are excited to offer our medical facility services at a price that you can count on. We provide dedicated family practice Nurse Practioners, comprehensive diagnosis solutions, and a medical facility that is both inviting and comfortable. Our staff sees patients from five years old to one hundred and five! We offer medication management, diabetes care, yearly physicals, lab services, STI testing and counseling, as well as dietary and weight management consultation.
We are now able to offer home health, assisted living and nursing facility visits for our patients unable to make it into the clinic. At this time, we can provide full service out-call visits for Medicaid, and medication management only for Medicare patients. For more information, please call us at (503) 449-8988.
Unfortunately, we are no longer providing skilled nursing home or facility visits; please feel free to inquire in the future as we may be able to offer that service again soon!
